SURGICAL RESECTION IN CANCERS OF AMPULLA OF VATER

Abstract
Carcinoma of the ampulla of Vater is a relatively uncommon neoplasm. However, patients with ampullary tumors have more favorable prognosis and the 5-year survival rate is better than those with pancreatic or bile duet turnors. in this study a retrospective evaluation of 14 patients who had the diagnosis of ampulla Vateri carcinoma between the years of 1991-1998 was performed, Eleven out of 14 patients had pancreatoduodenal resection (PDR) and local ampullary resection (LR) was performed in the remaining 3 subject. There were no complications in the LR group but the PDR group had a complication rate of 45%. The mean survival periods in PDR and LR groups were 24 and 20 months respectively. AH of the patients with LR developed recurrence. When neoplasms are determined in the ampulla of Vater, PDR is the procedure of choice when resection is possible. Indications for LR are limited. Although this procedure is recommended for small and probably benign tumors in-patients having malignant tumors with a high operative risk when technically possible however, this procedure has a high recurrence rate.
