IS BOWEL PREPARATION NECESSARY FOR PRIMARY RESECTION AND ANASTOMOSIS OF THE MALIGNANT OBSTRUCTIVE COLON LESIONS?

Abstract
Surgeons are always under stress during their approach to colon pathologies because of the multiplicity of the alternative operations in emergency, colonic flora and the high mortality and morbidity rate. The aim of this study was to evaluate the effect of bowel preparation on the primary resection and anastomosis (PRA). The early results of PRA in 19 emergency cases with PRA in colon obstruction who have tumors above the peritoneal reflection compared with 33 elective patients whom Nichols bowel preparation were performed between the period of 1990-1996 at Ankara Numune Hospital. The groups were matched according to age, sex, tumor location and Dukes classification. Although colon tumors which causes obstruction have higher risk for PRA, there were no statistical differences among the complications such as anastomotic leakage, intraabdominal sepsis, relaporatomy, postoperative hospitalization and mortality, compared to the elective group. We believe that PRA can be performed safely without bowel preparation unless the patients has advanced colon tumor, generalize peritonit and high medical risks.
