WHAT SHALL BE THE APPROACH TO INCARSERATED GROIN HERNIAS? COMPARISON OF ANTERIOR AND PREPERITONEAL TECHNIQUES

Abstract
There are a variety of surgical approaches for acutely incarcerated groin hernias. The aim of this study was to compare anterior and preperitoneal approaches to the acutely incarcerated groin hernias.
Ninetytwo patients, who were operated at the Emergency Surgical Clinic of Ankara Numune Hospital between September 1993 and May 1995, were retrospectively reviewed. In 64 patients (Group 1) anterior approach and in 28 patients (Group 2) preperitoneal approach was the choice of herniorrhaphy.
In group 1, there were 4 intestinal perforations, 3 inadequate exploration of the sac and one bladder perforation in these patients 8 additional insicions were made for therapeutic intent while it was unneccessary in group 2.
Preperitoneal approach gives a chance of better exploration of the abdominal cavity and sac and also it reduces the need of additional incision. For these reasons it is a good surgical approach to the acutely incarcerated groin hernias.
