HEMODYNAMIC EFFECTS OF CARBONDIOXIDE INSUFFLATION IN MINIMALLY INVASIVE SURGERY

Abstract
In this study, 20 patients without any accompanying disease who underwent laparoscopic cholecystectomy are evaluated for the increased intraabdominal pressure due to CO2 insufflation and its hemodynamic effects. Thoracic electrical bioempedance was used for hemodynamic monitorization and the maximum intraabdominal pressure was 18 mmHg or less. Important decreases were measured in left cardiac work index, cardiac index, stroke index, end diastolic index, contractility index and acceleration index. Elevations of mean arterial pressure and systemic vascular resistance index was detected.
In this study, we found that intraabdominal pressure elevations can induce important hemodynamic changes. For this reason in patients with cardiac problems, hemodynamic parameters must be closely monitorized and measures should be taken to decrease the risk of increased intraabdominal pressure intraoperatively.
